Embassy of Lebanon, which used to be called Lebanon Health Care, sits at 700 Monroe Rd in Lebanon, OH, and is tied to Embassy Healthcare, and sometimes it's called a Skilled Nursing Facility, and right now, they aren't taking new patients, which is probably important to know if you're looking for a spot because people ask about that sort of thing, and you'll find English-speaking staff there who come from all kinds of places including Aspen Rehab, with RNs, LPNs, nursing assistants, and therapists always around any time of day or night, which matters for folks who need regular help with things like getting dressed, bathing, eating, taking medicine, or just getting to the bathroom. Everybody there seems to have some type of support, whether it's long-term care, short-term care, memory care-including Alzheimer's and dementia programs-or all different kinds of therapy, including physical, speech, and occupational, which they run every day to try and help people keep or get back things like balance, moving around, or thinking skills; and they mention special services like wound care, post-surgical rehab, pulmonary, diabetes, cardiac, IV therapy, hospice, respite, neuro-muscular, pain management, stroke, and even renal care, with a focus on helping people get stronger or ready to go home if that's the next step for them, and they're organized enough with discharge planning to make the switch easier. There's a special Pulmonary Unit for people with breathing problems who need more attention so they can try to stay out of the hospital, and all the treatment plans get put together and checked on by the care team as the days go along. Private and semi-private rooms are options, and there's mention of a patio, a courtyard, cable TV, and phones for people who want those sorts of things, and you get a sense they want it to feel a little like home, even though it's still a place for medical care, so while you won't find a lot of fancy names or new-age activities listed, the main thing is they've got steady healthcare services like skilled nursing, rehabilitation, memory care, nutrition, orthopedic, outpatient therapy, subacute, and even adult daycare, assisted living, and independent living, depending on what anyone needs at different times, and although not everything is spelled out, it looks like there's a big focus on covering needs from short to long term, with plenty of caregivers around to give help and keep folks comfortable as best they can.

Compassionate staff but administrative issues
I'm conflicted. The caregivers, therapists (PT and respiratory), nutrition team and housekeeping were compassionate, skilled, and made my loved one comfortable - staff like Elizabeth, Andrea and Nikki truly went above and beyond; the dementia and pulmonary programs, meals and cleanliness were excellent. But administration and clinical coordination were inconsistent: understaffing, delayed medical responses (bloodwork, diaper/continence care), poor communication, billing/visitor problems and occasional safety concerns undermined care. I'd only recommend this place cautiously - check state records and overnight/incident reports before placing a loved one.
