Lima Convalescent Home sits on a campus where elders can find different levels of care, including skilled nursing at The Greens and the main building, as well as assisted living at Lochhaven Apartments with one or two-bedroom layouts, so family members get to choose what fits best, and it's good to know the place is non-profit and run by a volunteer board of trustees who live and work there, which brings a community feeling. The facility offers help with daily needs like dressing, bathing, and toileting, along with medication support, three meals a day served restaurant-style, snacks, and housekeeping, and the certified dietician plans the menus. There's a secure Memory Care Unit for people living with Alzheimer's or other dementia, and they get safe spaces, individualized care plans, and daily activities designed for memory care, while the special-care units have a permanent nursing staff and extra safety features, including an emergency call button in each apartment and visitor screening at the door with an intercom system. The Rehabilitation Center helps residents recover from health issues with physical, occupational, and wound care therapy, and nurses work together with doctors to plan care for different needs, including short-term stays or extended care if someone needs more help than can be provided at home. There's transportation for appointments and outings, fitness and wellness programs, and outings to local events, with activities like games, cooking classes, outings, movies, and arts and crafts-plus there are walking paths, outdoor areas with trees, a fireplace in the living room, social spaces, an activities and game room, computer room, library, WiFi, cable TV, and phone service. The building's fully sprinklered for fire safety and is handicap accessible, there's 24-hour nursing support with about 0.73 registered nurse hours per resident, regular wellness checks every two hours, and safety checks are based on quality indicator surveys to prevent accidents, while the whole campus keeps a neighborhood feel. Residents can join family or resident councils, use the on-site beauty and barber shop, enjoy private rooms with baths, relax in the remodeled west wing hearth room, and bring their pets, all while having access to laundry, maintenance, services, scheduled tours, waiting lists, initial assessments, and all the expected support, and since the home accepts Medicare and Medicaid, it makes the care more affordable for families who need those options.

Great rehab but staffing concerns
I love the residents and the gorgeous grounds - many staff are sweet, caring and hardworking and the rehab services are A+. That said, they're often short-staffed and some nurse's aides are grumpy and undertrained, so residents don't always get the care they deserve. Be very aware before placing a loved one - I wouldn't send family without careful vetting - but as someone connected here, I personally wouldn't go anywhere else.
