Twilight Gardens Nursing & Rehabilitation sits in Norwalk, Ohio and has 99 certified beds, and while it offers many care services for both short-term and long-term residents like skilled nursing, memory care for dementia and Alzheimer's, physical, occupational, and speech therapies, plus respite care if a caregiver needs a break, it's had several state inspection reports over time, with the last one from October 19, 2023 showing 6 deficiencies, including some issues about activities of daily living and nutrition needs, all marked as having no actual harm but showing potential for more than minimal harm, and there have been 17 deficiencies cited altogether, with 4 of those related to infection control, so that's worth knowing if infections are a concern for anyone considering this place, since sometimes those things indicate areas that need improvement even though the facility does focus on providing care and support. Staffed with a nurse turnover rate of 45.9%, which is pretty high, Twilight Gardens reports total nurse staffing hours per resident per day of 4.27, which helps give an idea of how much time nurses spend with each person, but some might want to ask about that if nurse familiarity is important. You'll find a team of healthcare professionals there who make individualized care plans aimed at promoting independence, dignity, and comfort, offering services like diabetic care, incontinence care, chronic disease management, and specialized non-ambulatory care for those who need it, on top of having an on-site Dialysis Unit for people who need kidney care and a Ventilator Program for those who need help with breathing. The facility has private rehab suites for those staying short-term, newly renovated spaces, and several shared indoor communal areas plus a beauty shop for residents, meals are provided for all, and folks can join in on social, recreational, and devotional activities both on-site and off. Amenities include WiFi, in-room phones, flat-screen TVs with cable, and the care staff aims to create a supportive environment focused on resident well-being. Operated by Cch Healthcare and guided by the principles of Compassion, Dignity, and Unity, Twilight Gardens addresses different health situations with personalized care and has special programs for rehabilitation and chronic conditions, but with its mix of both positive features like its therapy resources and on-site dialysis and some areas noted for deficiency, it's good to ask questions about recent improvements and stay mindful of all inspection results while considering this senior living option. Compassionate staff but systemic failures
I've seen both kind, attentive staff (some nurses and therapists really helped) and times when care was negligent and uncaring. Response to call lights and after-hours coverage was frequently slow, meds and treatments delayed, and I witnessed unsafe incidents - bedsores, infections, severe dehydration and an unresponsive resident who needed hospital transfer. Cleanliness and food were inconsistent: parts felt very clean and welcoming, yet I also found filthy rooms, soiled linens and cold, inedible meals. Communication and management were poor - unanswered calls, confusing visiting/discharge rules, billing disputes and little empathy from administration despite occasional improvements. I'm grateful for compassionate caregivers, but systemic problems make me cautious about recommending this facility.
