Sanctuary Wadsworth serves seniors who need skilled nursing or rehabilitation care, whether for a short stay to heal between the hospital and home or for long-term support, and it does so with a friendly, faith-based approach that welcomes people of all backgrounds, though it's rooted in the Catholic tradition and emphasizes spiritual healing for those who want it. The facility provides services for both men and women, and the staff includes experienced healthcare professionals who work together in teams to create care plans for each person, looking after health, spiritual needs, and comfort. You'll find specialized care here, including in-house dialysis in a dedicated den for up to 8 residents, five days a week, as well as ventilator support, tracheostomy care, wound care, and respiratory therapy, all in a setting that really tries to be comforting and respectful. Frail residents who need help with everyday life or support with more serious medical needs can get nursing and intermediate care, and help continues for people recovering from surgery, injury, or illness. Sanctuary Wadsworth also supports residents and families with hospice care, referrals, and a calendar of activities-details of which are posted on the Resident Life page-plus spiritual programs and ministry led with respect for everyone's traditions but grounded in Catholic values about dignity and the sanctity of life. The environment is warm and tries to pay attention to each person's individuality, and both physical and spiritual care get equal weight. The facility has earned a 5-star rating from the Centers for Medicare & Medicaid Services (CMS), and its holistic, inclusive culture helps residents feel part of a real community while getting the care they need.

Compassionate staff, management and safety
My experience was mixed. The nursing and therapy teams were exceptional - compassionate, patient, family-like, and great at rehab/OT/PT; the building is generally clean and many residents were comfortable. But administration was often rude, restrictive, and unprofessional (visiting/parking limits, denied building access), staffing was inconsistent with high turnover, and I witnessed serious care lapses (missed meds, hygiene/catheter issues, delayed ER responses) that felt neglectful. I'd recommend this facility for short rehab stays because of the staff, but be very cautious about long-term placement and watch for management red flags.
