    Severe neglect at city facility

I placed my mother in this city-run senior living facility and regret it. Despite a few good nurses, she experienced neglect: a heel cut turned gangrenous and led to amputation, she lost significant weight from poor eating assistance, suffered bruising, and personal items (clothes, watch, blanket) went missing. Administration has been evasive about money owed and I'm pursuing legal action - I will not associate with or recommend this place.

    About Four Seasons of Washington

Four Seasons of Washington is a 60-bed skilled nursing facility in Ohio, run by Ms. Tracy Ross and Mr. Tim Ross, and has business details registered with the Ohio Department of Health, so you know it's checked by the state and they keep their inspections up with an A- rating, which isn't perfect but shows there haven't been any severe issues. This place covers a wide range of care, offering everything from independent living for seniors who want to stay active to assisted living, skilled nursing, personal care, palliative care, memory care for those with Alzheimer's and other dementias, respite and hospice services, and even home caregiving, so folks in pretty much any situation can find support. The memory care unit is secure to help keep residents safe and prevent wandering, while specialized programs for mind-stimulation, individualized wellness, and reducing confusion make life gentler on those who struggle with memory loss. The staff includes registered nurses, LPNs, and CNAs, all trained to give 12-16 hours of hands-on nursing care each day, with 24/7 call systems if someone needs help overnight, and these clinicians also handle activities of daily living like bathing, dressing, transferring, and medication management, while offering meal preparation following special diets, plus a chef making sure food tastes like home. They have programs for pain management, diabetes, orthopedic rehab, mental health, and nutrition, and offer speech therapy, wound care, IV antibiotics, hemodialysis, and bariatric care, which make it easier to handle tough medical needs without leaving the community, so families don't have to move their loved ones around. There's outpatient and inpatient rehab for people getting back on their feet after illness or surgery. Amenities include furnished rooms with private baths, cable, Wi-Fi, kitchenettes, air conditioning, and phone service, and there's help for folks who can't walk on their own as well. For daily living, Four Seasons of Washington has emergency alert systems, transportation for medical visits, parking, and homemaking services, which help people stay comfortable and safe. Community life stays busy with activity rooms, a game room, movie theater, arts and crafts, a spa, library, outdoor paths, and a garden for quiet times, while fitness and wellness programs, scheduled daily activities, movie nights, music events, and outings help people stay connected and make friends. The staff pays a lot of attention to supervision and health, especially in providing thorough rehab and regular medical care, working together as an interdisciplinary team to support smooth moves in and out of the facility. Folks can use private insurance, Medicare, or Medicaid for payment, which helps keep care affordable for most families, and the place is open at all hours, so help's always available. Four Seasons of Washington stays focused on offering a high quality of life, and while it covers many areas-like Alzheimer's care, rehabilitation, nursing care, independent living, and more-it doesn't act like life always goes perfectly, but they do focus on comfort, kindness, and helping each resident reach their best possible health, whether the stay is short-term or long-term, and the place might not be fancy but it does keep things running safe, helpful, and respectable for the people living there.
