Willow Haven Healthcare & Rehabilitation sits in Zanesville, Ohio, on a big campus that feels a little like the countryside, with 81 rooms offering mostly semi-private living for an average of 76 residents each day, and folks there will find both short-term and long-term care plus a wide range of health services, though the place does have a much below average rating for health inspections, staffing, and quality, and a slightly below average overall score according to CMS, which means there are some concerns, especially since recent inspection reports found three infection-related issues. The staff includes licensed nurses, therapists, dietitians, social workers, and medical directors, and they do provide skilled rehabilitation services for people who need inpatient or outpatient care, including physical, occupational, and speech therapies, and they also offer help with nutrition, recreation, hospice, post-acute stays, respite, and more, and while there's a beauty parlor and barbershop on site and activities for folks to join, it's important to know the nurse turnover rate is pretty high at 50.7% and nurse staffing hours come out to about 2.98 per resident per day, a rate that's lower than many other places. The facility is operated by Certus Healthcare, a for-profit company with several locations around Ohio, and they do accept both Medicare and Medicaid, but it should be noted that right now Willow Haven isn't taking new patients. Recent inspections found some problems about honoring residents' rights, including how residents are allowed to voice concerns and keep their communications private, so while staff members are described as compassionate and they do work to tailor care to individual needs, these issues are something families and residents should be aware of. The place has semi-private rooms and a variety of care plans, and they say they're working on improvements with renovations and some upcoming events, plus there are online updates for provider directories and COVID-19 notices. The continuing healthcare programs at Willow Haven include regular medical care, therapy, and administrative support, so while there are plenty of services under one roof-ranging from skilled nursing, rehabilitation, and social activities to specialized programs and care for people with different needs-it's always a good idea to review the inspection reports and ratings to get a clear picture before making any decisions.

Mostly positive with inconsistent nights
My family member is a resident and I've been mostly impressed - management and many staff are friendly, caring, and hardworking (excellent activities, clean facility, attentive nursing/therapy), and it feels very family-like. That said, nights/after-hours care can be inconsistent (unanswered call lights, some rude/agency staff and reports of neglect), so ask about third-shift coverage. Overall I'd recommend it with that caveat.
