Penn Highlands Jefferson Manor sits at 417 Route 28 in Brookville, Pennsylvania, and you'll find they take care of seniors who need different kinds of help, like skilled nursing, independent living, and personal care, so if someone needs short-term rehabilitation or everyday support, they've got those services in the same place, with a total of 160 certified beds and a focus on making things feel homelike even while providing medical care. Staffed by a skilled team, they deliver around-the-clock nursing and offer therapies like physical, occupational, and speech, and there's also memory support available in secure areas, which helps residents who need extra safety and routine. The whole place is operated by Penn Highlands Healthcare, with managerial oversight from Brookville Hospital and Dubois Regional Medical Center, so it's run by an organization rather than a private owner or chain, and they're a non-profit corporation, so the focus is more on care than making money. They follow rules on infection control and residents' rights, including letting people refuse treatment or take part in research if they wish, and they put a lot of effort into creating a respectful, compassionate atmosphere, though an inspection has noted an infection-related deficiency in the past. They provide care planning and work to help residents keep a good quality of life, with privacy practices and a non-discrimination policy in place. You can reach out by phone, email, or online at any time for referrals, since they take them 24/7 from doctors, families, or anyone helping a patient, though their main office hours run Monday through Friday, 8:00 to 4:00. The nurse turnover rate comes to 46.2%, and nurses provide about 3.66 hours of care per resident each day, and while the exact amenities or activities aren't listed, the emphasis stays on safety, comfort, and reliable medical and personal care for every resident living there.

Inadequate staffing, poor care, overpriced
I moved my mother out after a short stay because staffing was inadequate (often one LPN for 26 residents) and nurses were even leaving the floor to open locked doors after 5:00. The food was inedible and menus weren't appropriate for sick elderly residents; housekeeping and activities were largely stopped, her belongings were dirty and treat bags taken. Management was unsympathetic, strict, and charged astronomical rates while treating staff poorly during COVID. The one bright spot was rehab - PT/OT/speech helped her speech and some floor staff were caring and supportive. Overall I would not recommend.
