    Friendly staff, unsafe/uncaring management overall

    I'm torn: the frontline staff - nurses, rehab therapists and activities teams - were friendly, skilled and helped with real improvement; the facility was often clean and the day-to-day care and social programs were excellent. But management and medical oversight were seriously lacking: blocked visitation, unanswered phones/long holds, understaffed/overworked nurses, poor communication, ignored family concerns, spotty medical follow-up (we filed complaints with the Dept. of Health and CMS and even pursued removal of a resident). Overall, great staff but uncaring management and safety/communication problems make me hesitant to recommend for long-term care.

    Mountain Laurel Healthcare and Rehabilitation Center sits in Clearfield, Pennsylvania, right in a rural community, and this very large nursing home has been serving people since 1965, with a focus on both long-term care and short-term rehab, and you'll find it's been set up to help both those needing skilled care after being in the hospital and those living with memory problems like dementia or Alzheimer's. The place can take in as many as 240 people but usually has around 120 residents each day, and you'll see a secure memory care unit so folks who need extra safety and attention because of memory loss can get what they need. The staff includes registered nurses working different shifts, with nurse staffing at about 3.63 hours per resident each day, and the nurse turnover rate has been 37.7%, so there's some change in who you'll see. Mountain Laurel can provide both inpatient and outpatient rehab services, with a full team for speech, physical, and occupational therapy, and there's care for strokes, medication management, tracheotomy, IV antibiotics, wound care-including a wound care nurse daily and a wound care doctor each week-plus respiratory therapy and pain management. Respiratory services cover things like oxygen, nebulizers, and help with sleep apnea, and the place also treats wounds ranging from simple ones to stage four with vacs.

    Residents have help with everyday needs like bathing and dressing, medication, and meals, and the place takes care of non-ambulatory folks too. There are respite stays to give families temporary relief, and the center takes Medicaid and Medicare, so there's help with costs for many. Some other things offered include pharmacy, specialized diets like diabetic-friendly meals, and meal prep done by a professional chef, plus dining takes place in a community room with allergy-sensitive options and restaurant-style service. You get laundry, housekeeping, moving help, family support, and transportation as part of the general services, and parking's available if visitors need it. Around the home, there's a focus on keeping residents active, with a modern fitness room, a wellness and spa center, outdoor spaces, a garden, and walking trails. Inside, rooms offer private bathrooms, air conditioning, Wi-Fi, cable TV, telephones, and kitchenettes, and everything comes furnished.

    It's worth knowing the place gets routine inspections, and it's had a historical overall grade of F for care and safety, although as of the last inspection it scored a B-minus, and there've been 73 total deficiencies found over time-including three tied to infection control, and some related to not always meeting the needs or wishes of residents and not always delivering trauma-informed, culturally aware care. The center's tried to build a sense of community, with regular events, music, art classes, movie nights, and resident-run activities, plus outdoor programs and a library, arts room, and game space. A resident council lets people voice concerns and work on changes, and the staff tries to build care plans with input from both patients and families. Mountain Laurel is for-profit, owned by Mountain Laurel Opcp Holdco LLC, with some indirect owners listed as Bl Panho Trust, Mlspa LLC, Mlspa Trust, Mvrk I LLC, Panho Capital Group LLC, Andrew Garbacz, Daniel Gottesman, and Robert Kolman, and it's managed by Premier Management Support LLC since December 2021. The center's inspection record says they haven't always met every standard for quality or infection prevention, so for those looking at living here or bringing a loved one, it's wise to look at those reports and talk with current residents or staff about care and support before making a choice.
