Embassy of Park Avenue sits in Meadville, Pennsylvania, as a nursing home with 173 certified beds, and quite a few different care options like assisted living, independent living, memory care, long-term care, and skilled nursing, and it takes folks for both short and long stays, and you'll find nurses and care providers, staff trained in medication, and people who can help with mental health needs, too, so if someone needs wound care, diabetes or cardiac help, wound care, or even rehab like physical, speech, or occupational therapy, they're all available, plus there's pulmonary and orthopedic programs for those who need them, and they really do offer a bunch of health and wellness activities, special diets, and help with things like diabetic care or nutrition. Folks living there use shared common areas, eat meals together, and can join in games, hobbies, and other planned programs, and when it comes to getting around, they help with transportation and shopping, plus they've got home care, respite care, and even adult daycare. The staff counts over 200, with nurse turnover around 39.0%, and nurse staffing is about 3.39 nurse hours per resident per day, so while the place does strive for safety and keeps with federal infection control standards, there are 16 documented deficiencies, including one infection-related deficiency and others tied to equipment safety and resident rights, though none with actual harm but the potential for more than minimal harm, and the latest inspection's dated November 22, 2024. This place's managed by Embassy Healthcare, working under a for-profit model, with ownership through Embassy Pa Tenant 2 Holdings LLC and connections to a handful of other LLCs and trusts, and they have policies to protect privacy and follow HIPAA rules. People can set up scheduled tours if they're interested, and the community's got access to services and information through its social media pages, so families can stay up to date. The place puts time into specialized care plans, tries to get people home sooner, and works to reduce hospital visits, though some things, like management details, aren't public, and while there are areas for improvement, folks do seem to care about the residents' wellbeing and comfort, and that's worth knowing.

Outstanding therapy, inconsistent care, beware
I appreciated the beautiful, very clean facility and the outstanding rehab - the therapy team, gym and therapy dogs got me back home quickly. Many caregivers were caring, compassionate and communicative with my family. Unfortunately the food was often cold, late and unappealing, and there were frequent dietary, prescription and discharge hiccups. Staffing shortages, high turnover and a few unprofessional or uncaring employees made care inconsistent, so I'd recommend it for short-term rehab but be cautious for longer stays.
