    Spotless facility, inconsistent unsafe care

I had a mixed experience. I found the facility spotless, odor-free, bright and inviting, with many helpful, cordial staff and excellent rehab (PTs like Tracie and Savannah were outstanding), good activities and welcoming common areas. However, care was inconsistent - poor communication, safety and medication lapses, abrupt discharges and management/billing issues were real concerns, and I noticed staff seemed more attentive when family visited. Overall I encountered both compassionate, professional caregivers and troubling neglect, so weigh the pros and cons carefully.

    About Fountain Inn Post Acute

Fountain Inn Post Acute sits at 501 Gulliver St in Fountain Inn, SC, and serves people needing both short-term rehabilitation and longer-term care, and folks might notice right away that the place feels clean, home-like, and well cared for, and the staff work hard to keep things running smoothly, with environmental services keeping up with laundry, housekeeping, and safety standards so everything stays neat and fresh. The facility has 60 private rooms, each with its own bathroom and shower, along with large shared spaces where residents can gather and take part in things that help fill the halls with energy. Skilled nursing staff, including Registered Nurses (RNs), Certified Nursing Assistants (CNAs), and a Director of Nursing, provide round-the-clock medical care, and there's a Medical Director to oversee coordination with other doctors and specialists, so families don't have to worry about coordination slipping through the cracks. Folks who need help getting stronger or moving better can get physical therapy (PT), speech therapy (ST), and occupational therapy (OT) from a therapy team that creates personal recovery plans, and Restorative Nurse Assistants (RNAs) assist with daily activities to help folks keep or recover independence, which is something that becomes important over time.

There's a focus on providing care with respect and compassion, and staff build relationships with residents and their families. The leadership team, including a Licensed Nursing Home Administrator (LNHA), works together on operations, staffing, and services, so there's a steady hand guiding things. Dietary services, led by Registered Dietitians (RDs), ensure meals are nutritious and fit the needs of each person. Social programs, run by a Social Services Director and a team of social workers, help residents feel supported emotionally and socially. The facility, part of a network of 24 post-acute facilities in South Carolina, benefits from PACS Services, which gives the local team tools, consulting, and support to concentrate on patient care and quality of life.

The staff-including a specialized wound care nurse-are recognized for kindness, respect, and genuine care, and the environment allows for both privacy and socialization. The facility offers both skilled nursing and long-term care, as well as respite care for families needing a break, and they make sure everyone gets care that fits their needs, whether someone's there for recovery or to stay. Family members often comment on the engaged and passionate staff, and those who live here have access to a variety of activities and amenities that help meet their social and emotional needs. Skilled nursing is available 24 hours a day, and the staff want residents to feel they're in a safe and comfortable place that feels like home, and that's what the people there are working for every day.
