Chandler Creek Post Acute sits at 401 Chandler Road in Greer, South Carolina, where it serves as a post-acute care facility with 133 beds and a strong small-town feeling that many locals know well, since the staff creates an environment that feels as close to home as possible, and the building itself is set up with comfort and privacy in mind. Supported by PACS Group, Inc., which helps coordinate a network of 24 similar sites in South Carolina, this place gets extra aid from PACS Services so the care teams can stick to looking after patient and resident well-being. Bryan Tyner is the administrator, and the facility offers skilled nursing, rehabilitation services like physical therapy and occupational therapy, and a range of social services. Residents get personalized treatment plans, and they're encouraged to bring things from home to help with comfort during recovery. Care here focuses on meeting individual needs, especially after hospital stays or illnesses, and rehabilitation spaces are set up so people have what they need to improve. There's a regular staff of CNAs, LPNs, and RNs, with set PRN rates-$18 an hour for CNAs, $28 an hour for LPNs, and $33 an hour for RNs-plus shift differentials up to $4 an hour, and ongoing training available for nursing and ambassador (preceptor) roles, which means new staff can get training like DON training. Workers get employment benefits like up to $5,000 a year for nursing school loans, gym and activity reimbursements, weight loss program reimbursement, and paid time off for birthdays. Activities and social programs are part of daily life, there are English-speaking staff, and local connections help keep the place woven into the surrounding community. This facility is focused on rehabilitation, skilled nursing, and long-term care, with specialized services designed for seniors who need extra help during recovery or as their care needs change over time.

Mixed care, excellent rehab, issues
I had a very mixed experience. Some nurses, PTs and office staff were friendly, compassionate, knowledgeable, helped with Medicare/Medicaid, and rehab was excellent - they gave me real peace of mind. But other staff were inattentive, rude or negligent: messy/dirty rooms and wheelchairs, cold meals, medication errors, ignored call lights, slow/unresponsive administration, and even jewelry theft. Short-term rehab care I'd consider; for long-term placement I'd be very cautious and insist on answers and improvements first.
