Manna Post-Acute sits at 716 E. Cedar Rock Street in Pickens, South Carolina, in an older, well-kept building that looks clean and new inside and out, and you'll see a sign with the name near the entrance along with parking close by, which is convenient for visitors coming and going at any time since the place is open twenty-four hours a day for residents who need care around the clock. The facility has 130 certified beds, and on most days, about 123 folks live there, so it's never too empty or too crowded, just enough people to make company and to make sure care teams can look after everyone, especially since Manna Post-Acute partners with PACS Services to help caregivers focus on each person's well-being. Residents get a personalized approach that cares for the body, mind, spirit, and social life, with skilled nurses and a team providing care for short-term rehab, assisted living, and long-term support, and the facility also welcomes seniors needing help with chronic illnesses, wound care, physical, occupational, speech, and respiratory therapy, and IV therapy. The staff's nurse turnover rate sits lower than the state average, which helps with steady care and familiar faces, though the nurse staffing hours per patient are a bit below the average, something families may want to keep in mind alongside the thirteen deficiencies on inspection reports, some of which were infection-related or involved rights like dignity, communication, food, and fluids, areas the place continues to work on improving, as no place is perfect. Social services and activity programs help residents stay connected and active, and there's a back patio where people can sit outside and enjoy the fresh air when the weather's nice, plus hallways for easy walking around. While there have been some federal standard violations, the team, led by Administrator Todd Griggs, keeps the focus on restoring each resident's independence and dignity, encouraging personal comforts from home to create a place that feels more like home than a hospital. Manna Post-Acute has a solid reputation for post-acute care in Pickens and stays committed to enhancing life quality with modern facilities, even if it means facing challenges or working on areas that need attention, always aiming for a safe, homelike stay. You can find more about the facility and services on their website.

Neglectful poorly managed nursing facility
I placed my grandmother here and while a few staff (Nurse Practitioner Ruthie, social worker Ally and therapists Brittney and Kevin) were caring and skilled, the facility is poorly run. Staff were often on their phones, phones at the front desk rang endlessly, nurses were hard to reach, call lights ignored, communication was terrible, and the place smelled; food was awful. Delays in meds, tests and injury reporting (a resident fell and broke a femur), COVID handling, and unresponsive administration made me fear neglect and unsafe care. Given the safety lapses and inconsistent staffing I would not recommend this facility.
