Clean, caring staff; communication concerns
I placed my grandma here and overall I'm pleased - the facility is very clean and home-like, activities and therapy are excellent, and many nurses/CNAs (Amanda, Sidney, Adam, Cory, Michael and others) went above and beyond; meals and snacks were good (grilled cheese was a hit). That said, I saw serious staffing and communication issues - nights can be short-staffed, nurses' station unmanned at times, delayed care, locked bathrooms, and occasional unresponsiveness that raised neglect concerns. Overall positive for rehab and long-term care, but I'd recommend confirming staffing/communication practices before committing.
