Golden Skyline Assisted Living sits at 76 N 100 W in Ephraim, Utah, right near the corner of N 100 W and W 100 N, and it's a medium-sized community with 16 licensed beds. This place focuses on assisted living and long-term care for seniors, providing support for folks who need help with daily life but want to keep their independence for as long as possible, and the staff work closely with each resident to make sure their needs are met through personalized care plans. There's always someone on duty, with certified nursing assistants and registered nurses handling care, medication, and medical monitoring, and the nurse call and emergency system help everyone feel a bit safer. They've got several studio room layouts, including couples suites, deluxe suites, and standard suites, all meant to be comfortable and private-there's no room sharing, each room has either a private half- or full-bath, some with walk-in showers, and the couples suite has its own kitchenette. The days run smoothly with three home-cooked meals plus snacks and treats, which dietary professionals check for good nutrition, and the kitchen staff take pride in making meals fresh. Golden Skyline provides more than meals and care-they've got housekeeping, laundry, transportation for limited medical appointments, and all the standard support with things like bathing, eating, dressing, toileting, grooming, and getting around, plus 24-hour supervision and safety monitoring with cameras. Social and recreational programs happen through the week, with art classes, gardening, movie nights, exercise sessions, and other activities that keep everyone feeling like they belong. Residents can join adult day care, respite care, and transitional care as needed, and regular checks help keep up with changing needs over time. There's a large courtyard, a library, TV and WiFi in every room, and a lounge where folks gather. The team shows care in everything, trying always to treat people with respect and help them keep their dignity even as their health changes. Golden Skyline doesn't take Medicare unless recognized by the Centers for Medicare & Medicaid Services, but they offer an all-inclusive monthly fee covering meals, personal assistance, and activities, so it's easy to know what to expect. The whole place, from the private rooms to the caring staff, is set up to feel homelike and welcoming, with a bit of extra support for each person as they need it. Modern, clean, loving, professional care
My family member has lived here six years and I couldn't be happier. The staff (Cameron especially) are professional, loving and treat residents like family; management and owners are active and attentive. The facility is modern, spotlessly clean and safe, with CNAs and a full-time nurse on site, individualized one-on-one care, fresh home-cooked meals (no powdered eggs), a fenced garden and busy, appropriate activities like BINGO. Affordable, reassuring, and highly recommended.
