    Great caregivers, poor communication issues

I'm torn: Seasons has a clean, fresh facility with private rooms, home-cooked meals, strong therapy/rehab, many engaging activities, and genuinely caring staff (Jeremy and others kept our family informed during COVID). Too often, though, communication around admissions and an undisclosed age policy disrupted our plans, transfer paperwork was rejected, and we were left frustrated by delays (including a delayed refund). Some employees and managers were compassionate and skillful, while others were rude, dismissive or slow to respond-I experienced what felt like gaslighting and intimidation and had to constantly advocate. Staffing shortages led to missed call lights and care lapses for a few residents, so vigilance is needed. Overall I'm grateful for the excellent caregivers and clean environment, but proceed carefully and document everything.

    About Seasons Health & Rehab

Seasons Health & Rehab sits as a 53-bed, privately owned facility that provides skilled nursing care, rehabilitation, memory care, and long-term care alongside assisted living, independent living, and adult day care options. Residents can get help with bathing, dressing, grooming, walking, toileting, and daily activities, and there are Certified Nursing Assistants always there to assist with personal care and getting around. The staff handles wound care, medication support, physical therapy, occupational and speech therapy, hospice care, and has both a Medical Director, Dr. Grayson Dart, D.O., and an Assistant Medical Director, William Tobler, PA-C., overseeing resident health, with nurses on-site at all hours. Special programming and individualized care plans support those with dementia or Alzheimer's, and Infection Control Nurse Jeri Orr focuses on safety, especially with recent reports noting infection control deficiencies, showing the facility pays attention to health standards and works to address issues.

Family members and residents often talk about their care needs with the clinical team, which creates personal care plans and discusses things like payment methods, Medicare, Medicaid, long-term care insurance, costs, contract terms, and what service options fit best. The management comes from Traditions Health Care, Inc. (since 2018), and direct ownership is held by Canyonlands Health Care Special Service District. The nurse staffing hours average about 3.84 hours per resident per day, and the current census runs around 45 residents daily, with 75 staff members including Lisa Panici as Office Manager, Marty Maggard as Maintenance Manager, Jacob Martin for Transportation, David Gray as Occupational Therapist, Carl Green as MDS Coordinator, Russ Tolbert as Physical Therapist, Tami Allen as Recreation Therapy Manager, Yanely Santos for Dietary, Maria Martinez in Housekeeping, Terral Bassett as Director of Nursing, Sarah Kelley as Resident Services Manager, Wil Heimuli as Assistant Director of Nursing, and Jeremy Frehner as Administrator.

Residents can live in various room types such as studio, single room, two-bedroom, semi-private, and private rooms, all set up with safety and handicap features, sprinkler systems, cable TV, and Wi-Fi. There's guest parking, washer and dryer access, kitchenettes, and a fitness center along with property areas for recreation like game and activities rooms, a salon/barbershop, outdoor spaces for sitting or walking, and scheduled transportation for medical visits or errands. The dining service is well-reviewed by residents' families, offering menu options with meals served in dedicated dining rooms, and regular cleaning helps keep the place fresh. People staying here can join in on social events, arts and crafts, learning programs, and health or wellness groups led by both therapy and recreation teams, including outings helped by Recreational Therapists, which keeps the days interesting and supports mental, physical, and social well-being.

Seasons Health & Rehab tries to give everyone a comfortable, supportive, and safe place to live, with a strong focus on privacy and respect for residents' choices, and promotes understanding the differences between independent living, assisted living, memory care, and skilled nursing through resources and tips for families and caregivers. The facility uses modern healthcare tech, supports different payment types, and gives tours-either in person or virtually-so families can review services and amenities and talk through any questions. Seasons is recognized with accreditation and has received commendations for care, and its full calendar, the activities, and day services all work together so people can feel active and involved throughout their stay.
