    Beautiful facility, great therapy, concerns

I'm torn - the building, activities and dining room look great and the PT/OT team (they rebuilt strength and balance) and many nurses/CNAs were compassionate and effective. But staffing and management are inconsistent: rude or undertrained aides, long call-light waits, phone/communication failures, medication/wound-care mistakes and occasional neglect/safety issues. Food, housekeeping and responsiveness were hit-or-miss, and high turnover/money-driven decisions undermined trust. In short: excellent therapy and a beautiful facility, but serious staffing, safety and communication concerns mean I'd be cautious about long-term placement.

Rocky Mountain Care - Willow Springs sits in Tooele, Utah, close to I-80 and SR-36 for easy access, and it's part of Rocky Mountain Care, with management from Rocky Mountain Care LLC and Rocky Mountain Care Tooele, Inc. since April 2013, and the specific managers there now are Edward Bangerter, Raymond Beeman, Laura Boardman, Fredrick Hale, Bradley Mikesell, Kristy Rehfeldt, Lance Samuelson, and Alan Widdison who've been listed in charge since various times starting in 2022 and 2023. The place runs as a non-profit, takes care of an average of 103 residents a day, and has 112 certified beds, though some sources list 104 total beds, with both private and semi-private rooms, and there's hospice and home health on site. Residents can get skilled nursing, respite, short-term stays, rehab, dialysis, physical therapy, occupational therapy, speech therapy, bariatric care, intermediate care, and even have access to a specialized Alzheimer's wing. Applicants can use private pay, private insurance, long-term care insurance, Medicaid, Medicare, and managed care benefits here.

You'll find a full-service salon, Wi-Fi everywhere, a computer nook for visitors and family, a common television area with DVDs and fresh popcorn, private rooms with big private baths and walk-in showers, plus extra-wide beds with pressure-relieving mattresses, and a separate ice cream parlor giving an old-fashioned feel, since they've got their own "Center and Main Streets" inside. Meals come from an award-winning chef, with fine dining, flat-screen TVs from 32" to 42" and DirecTV, a fitness center filled with updated therapy equipment, wireless high-speed internet, and a team of RNs, LPNs, and CNAs who are there 24/7 for care, as well as a specific focus on transitional rehab and chronic care management.

The nurse turnover rate here sits at 48.6%, with nurses averaging 3.70 hours per resident daily. Nurses and aides work under licensed, caring staff, and local nurses, therapists, and the office make personalized wellness plans for each person who comes to stay. Rocky Mountain Care - Willow Springs has won the Activated Insights 2025 Customer Experience Award™ for good cleanliness and professional therapy. The facility does round-the-clock nursing and skilled care, outpatient therapy, palliative and personal care, and has emergency pendants and physician services. There are some deficiencies noted on inspection, including issues with accident hazards, medication error prevention, how the staff responds to claims of abuse or neglect, and infection control, which means people using the facility should stay informed and aware when choosing care here. Overall, Rocky Mountain Care - Willow Springs offers several services and a few unique touches to support the health and quality of life of its residents.
