Accordius Health at Roanoke sits in a historic, scenic part of Roanoke, VA, and serves folks who need skilled care and support after things like illness, surgery, or hospitalization, and while the building itself holds up to 130 beds, only 60 are certified for Medicare and Medicaid as of June 2025, and 18 are set aside for vent patients, which means people who need ventilator support, and this place isn't fancy in a flashy way but tries to focus on comfort, safety, and the dignity of each resident by offering things like emergency alert systems, 24-hour supervision, a call system, and security features along with private bathrooms, air conditioning, cable TV, kitchenettes, Wi-Fi, and furnished rooms so each person can settle in more easily. Nursing care runs between 12 and 16 hours a day, with staff that includes registered nurses, licensed practical nurses, certified nursing assistants, social workers, and dieticians, and the care team helps with daily things like bathing, dressing, transfers, and medication management. The facility provides assistance with meals, family support, move-in coordination, and housekeeping and laundry, trying to keep it easier for those living there, and also offers special diets, including allergy-sensitive and diabetic options, prepared by a professional chef and served in a restaurant-style dining room or through all-day dining, which is helpful for folks who don't eat at set times. Community amenities include scheduled and resident-run activities-music programs, movie nights, arts and crafts, fitness classes, outdoor walking paths, and a garden where residents can spend time in the fresh air, participate in growing things, or just relax, and the community makes extra space for folks to hold game nights, visit the library or computer center, or get their hair done in the salon. Accordius Health at Roanoke goes beyond the basics by offering things like a mental wellness program, assisted living, and memory care, as well as personalized care plans put together by a team working closely with residents and their families during care conferences, and there are also resident and family councils so everyone gets a chance to voice concerns or suggest changes. The place is part of a Continuing Care Retirement Community (CCRC), which allows different levels of care as people's needs change, and there's also respite care for short-term stays if someone needs a little extra help for a while. Therapy programs at Accordius include physical, occupational, and speech therapy, and they've got an in-house hemodialysis unit, comprehensive respiratory care-including for ventilator-dependent residents-physician-led wound care, digital radiography, IV therapy, and specialized rehabilitation, like stroke and orthopedic rehab, with a focus on helping residents recover or maintain skills as much as possible. They run a teaching center for budding geriatric professionals in partnership with local universities, which brings in new people eager to learn and keeps the care up-to-date, and their structured activities and focus on outdoor space, a spa room, transportation services, and concierge help round out the experience so residents and families get a sense of security without being overwhelmed with options. The place operates as a for-profit Limited Liability Company, part of The Portopiccolo Group, and was sold to Curis as of May 31, 2018, and accepts both Medicare and Medicaid for those eligible.

Neglectful care despite good therapy
I had an extremely negative experience: staff were often rude or abusive, nurses unresponsive, call lights ignored or nonexistent (we used a hand bell and baby monitor), and residents were left wet, dirty, and unattended for hours - I saw bed sores, infections and multiple hospitalizations. Management was unresponsive, charts appeared falsified, the primary doctor was rarely present, and the facility seemed unprepared for new patients amid ongoing construction. There were alarming reports of unsafe conduct (including possible intoxication), no consistent COVID testing, and a generally unsafe, neglectful culture despite some diligent caregivers. On the positive side, the respiratory and rehabilitation/physical therapy teams were excellent and renovations (new floors, paint, courtyard) are improving the environment. Overall I cannot recommend this facility for nursing care - good therapy and remodeling don't make up for dangerous neglect.
