Shenandoah Center in Charles Town, West Virginia is a nursing home that has 138 beds and is operated by Genesis Healthcare, so folks will find a range of care options here, including skilled nursing, rehabilitation, memory care, assisted living, and independent living, all under one roof as part of a Continuing Care Retirement Community, which means people can move between different types of care if their health changes. Residents get help from a team of healthcare professionals at all hours, with both short-stay and long-term care available, and there are programs for those recovering from illness or surgery, as well as folks needing support for Alzheimer's, orthopedic rehab, ventilator care, dialysis, and palliative care focused on comfort and quality of life. The place has private and semi-private rooms, all air-conditioned with individual climate control, and there's a dining room with restaurant-style meals, special diets for allergies and diabetes, and extra services like dental care, wound care, and physiatry, plus colostomy and bariatric support, IV therapy, respite stays, post-hospital care, and hospice. The facility has a smoke-free policy, emergency alert systems, and easy access to registered nurses, attending physicians, and expert therapy, including physical, occupational, and speech therapies for anyone who needs them, which is good for those coming here after a hospital visit. Communal areas like arts rooms, walking paths, gardens, and a movie nights area give residents lots of chances to visit with each other, and there are barber and salon services, mail and newspaper delivery, and wireless internet throughout, with furnished rooms, cable TV, and security alarms for safety and comfort. The staff can help with daily activities such as bathing, dressing, and moving around, plus manage medications, meals, and move-in coordination, and there's housekeeping, laundry, and transportation options for appointments out in the community. Shenandoah Center also provides memory care for people with dementia, social services, a Veterans partnership, pet therapy, and activities that support religious, cultural, and recreational interests, so everyone has something to do. It accepts Medicare, Medicaid, and most private insurance, so families have different payment options depending on what's needed.

Compassionate staff, serious safety concerns
I found the facility clean, welcoming, and staffed by many compassionate, professional caregivers - excellent therapy, a helpful social worker, engaging activities, and clear updates/Zoom and window visitation options. However, I also encountered serious concerns: staffing shortages/turnover, inconsistent communication, and credible reports of missed care, medication/clinical lapses and neglect that affected some residents. Overall the staff often go above and beyond, but experiences vary - visit in person, ask about safety/incident procedures, and monitor closely before deciding.
