    La Crescenta Health Care Center sits as a for-profit, 92-bed skilled nursing facility, serving older adults who need help after a hospital stay, surgery, or illness, and it's got everything from medication management and help with bathing, dressing, and transfers, to 24-hour supervision and 12-16 hour nursing care each day, and with furnished rooms that come with Wi-Fi, air conditioning, private bathrooms, cable TV, telephones, and even a kitchenette, it tries to make day-to-day living comfortable. Residents find several community features-an arts room, library, fitness room, walking paths, movie theater, garden, and business room, along with daily scheduled activities, and there's a focus on keeping folks busy with movie nights, music, resident-run programs, and a variety of activities tailored to individual interests, all managed by a staff that includes a professional chef who prepares restaurant-style meals with options for allergies and diets like diabetes, with a registered dietitian on hand working with doctors for nutrition planning. The place takes both Medicare and Medicaid, has 88 average residents per day, and maintains four or so nurse hours per resident every day, so people who need help with personal care like bathing, dressing, and eating, or more advanced needs like wound care, intravenous therapy, pain management, enteral therapy, and both short and long-term care, can get it, and there's also a dedicated COVID unit with "dedicated heroes" staff for infection control, not to mention a strong emphasis on safety and communication with public health officials, and they've put in place visitation rules following health guidelines. All rooms have emergency alert systems just in case, and there's transportation, parking, housekeeping, laundry, and dry cleaning available, plus a Rehabilitation Therapy Department that tackles everything from basic tasks like cooking and cleaning, to dedicated occupational, physical, and speech therapies, and pain management. Inspectors found 63 deficiencies in recent reports, including 6 related to infection, and as of June 2025, 3 beds remain available; the most recent report on April 25, 2025, cited 12 deficiencies including issues with infection control and garbage disposal, so there are things to consider on that front. Staff work directly with residents and their physicians to create care plans, and offer psychological services, social and emotional support, restorative nursing, feeding assistance, and retraining when needed. La Crescenta Health Care Center does not operate inside a Continuing Care Retirement Community and is affiliated with Mariner Health Care, and while it provides a wide range of care and activities, it's wise to look closely at its inspection history and nurse turnover rate (37.3%) before making any decisions.
