    Spotless facility, caring staff, issues

I live here and have mixed feelings. The facility is spotless, the nurse is excellent and the CNA is good - many staff are friendly, caring, and give unrushed one-on-one attention. Dining and activities are OK (bingo/movies sometimes); food was better than I expected but not great, and they encourage movement with a PT center. My concerns: staffing is inconsistent (some lazy), I had a fall that wasn't monitored promptly and administrators didn't discipline staff, a haircut was done without consent causing distress, visits were denied during quarantine, and calls often went unanswered or hung up on. Overall I'm dissatisfied despite some genuinely good caregivers.

About Woodcrest Post Acute & Rehab

Woodcrest Post Acute & Rehab sits over in Riverside, California, where they run a nursing home and group home inside a Continuing Care Retirement Community. The place offers skilled nursing that covers rehabilitation, wound care, medication management, dementia care, and help for folks with complicated health needs, so if someone's just gone through surgery or needs therapy like physical, occupational, or speech, those services are right there. They've got a team working under Administrator Ms. Maria Constantin, and the nurses provide coverage, with an average of 4.77 nurse staffing hours per resident each day, but you do notice the nurse turnover rate's pretty high at 63.4%.

There are 120 certified beds here and they usually have around 97 residents in the building each day, with English-speaking caregivers, many of them. Residents get help with daily needs, like bathing, dressing, transfers, and medication, and folks who can't walk or need help moving also find support. You'll see a 24-hour call system in place, and nursing staff cover the building for about 12-16 hours at a time, though the support still runs every hour of the day.

If someone wants some extra help, they have social workers (LCSW), mental wellness programs, and family involvement is encouraged. The community provides care through respite stays, long-term care, end-of-life support, and post-acute rehab, and for those who need it, they offer things like IV therapy, diabetic management, pain management, TPN, and special arrangements for dental, vision, and audiology services.

Rooms come furnished, and most include private bathrooms, kitchenettes, cable TV, and air conditioning. Housekeeping, linen services, and meal plans adapted to each resident's needs and tastes are part of life here, along with activities like planned outings, games, day trips, a small library, and a garden, plus a beauty salon, wellness and fitness rooms, a gaming room, a computer center, and areas to relax inside and out. The team says they focus on comfort and compassion and try to make transitions easy, and they group together to build a friendly spirit. Transportation services-both from staff and by arranging outside rides-can help residents get around for non-medical outings.

Woodcrest has faced some concerns and deficiencies found in inspections. There have been complaints related to honoring resident dignity, communication, and personal choices, with 71 total deficiencies recorded, including two about infection control. The business is for-profit, not BBB accredited, and is affiliated with Nahs. Despite these concerns, the aim stays on offering support with comfort and recovery, while keeping health and safety as main priorities, using personalized care. They list their team on a "Meet the Team" page and there's a gallery if folks want to see the place before visiting.
