Autumn Care of Marshville sits at 311 W Phifer Street in Marshville, NC, and has 120 beds in a setting that's part of the Saber Healthcare Group network, and you'll find they offer both assisted living and nursing home services as well as short-term rehabilitation, skilled nursing care, memory care, and respite care, and they even provide long-term care for people who need it. The facility includes studio and suite rooms, all of which have private bathrooms, air conditioning, cable TV, Wi-Fi, telephone access, and kitchenettes, and everything's furnished so new folks can settle in easier, and families appreciate the 24-hour call system and emergency alert alarms for peace of mind. Residents get all-day dining in the common dining room with meals prepared by a professional chef, special diets like for diabetes or allergies are supported, and the staff provides help with meal preparation, laundry, dry cleaning, and daily housekeeping, which takes a load off for everybody, and the building feels home-like because that's what people say when they talk about living there. There's a concierge to help arrange things, move-in help is available, and the staff speaks English, so communication's clear, and a team of dedicated professionals handles medication management, help with bathing, dressing, getting around, activities of daily living, and even transferring for people who can't walk without help. There are 110 skilled nursing beds in the skilled nursing facility, and the staff supervises around the clock, offering up to 16 hours of nursing services and working with physicians to make custom plans, using rehab equipment and staged programs for both short-term recovery and longer stays, with physical medicine and rehabilitation available right on site. Folks can enjoy an arts room, game room, music programs, library, activity room, a small spa and wellness area, community spaces outdoors, and walking paths for fresh air, and the community plans monthly outings, bingo, hymn sings, and big events like the end-of-summer cookout, plus residents can join in crafts, fitness programs, holiday celebrations, and specialty groups that help make the days fuller. They host community-sponsored events, let people pre-register before surgeries for smoother transitions, and have an NRC Health profile for reviews and comments. Autumn Care's received recognition from the American Health Care Association in the 2020 Quality Initiative, with administrator Kimberly Josephs overseeing the team, though they don't list a public phone or fax number and aren't accepting new residents right now. Staff offer personal care and respectful support, working hard to create individualized plans that fit each person's needs, and while the community isn't for everyone, people who live or recover there have access to a lot of help and practical comforts in a place that feels steady, familiar, and attentive to both health and daily life.

Understaffed nursing, inadequate dementia care
The facility was clean, the staff I dealt with were pleasant and responsive, and my mom liked the food - but I do not recommend it. Nursing was low-quality and severely understaffed, dementia care was inadequate, there were long waits for bathroom help, and health issues weren't addressed; our loved one left COVID-positive, on oxygen, with systolic BP over 200. PT staff were excellent, but overall the stay was a difficult experience.
