Walnut Cove Health And Rehabilitation Center sits at 511 Windmill Street in Walnut Cove, North Carolina and has been open since 1974. The place has ninety certified beds, with six private rooms and forty-two semi-private rooms, and can handle up to ninety residents-sometimes more, sometimes less, but there's usually about eighty-one folks staying at a time. It offers long-term care, skilled nursing, and post-acute care around the clock for people who need help with health and rehabilitation. Folks get occupational therapy, physical therapy, speech therapy, and care for medically complex conditions, including support for Alzheimer's and dementia. There's a nurse on duty at all hours, though nurse hours per resident are 2.71 a day, which is lower than the North Carolina average of 3.8, and nurse turnover in the last year was 45.9%, a bit under the state average. The center's been a part of Consulate Management Company III, LLC since December 2021 and is owned by Centennial Healthcare Holding Co. LLC, making it a for-profit business that's linked to Consulate Health Care and other related care groups. Life at the center includes planned activities like garden time, craft making, movies, outings, group events, and visits from performers. Pastoral support and a beauty/barber shop are there, too, and there's a gym for rehab, plus spaces outdoors for fresh air. The center's got programs to help folks go back home safely, tailor-made care plans, and social activities aimed at helping people keep their dignity and live well. The staff tries to take care of the whole person-body, mind, and spirit. Couples can room together if needed. Over the last few years, inspectors reported both care and safety issues-there were deficiencies found with matters like accident hazards and supervision, including a fine of $8,827 from a July 2024 inspection and another fine of $16,801 from December 2024. The center had complaints and an infection-control report in the last thirty-six months. Walnut Cove Health And Rehabilitation Center calls itself an Ambassadors of Care facility and aims for personalized, compassionate care, but like all care centers, it's had its share of challenges along the way.
Walnut Cove Health And Rehabilitation Center
511 Windmill Street, Walnut Cove, NC, 27052- Skilled nursing
Neglectful, unsanitary, dangerous nursing home
I chose this place because it's close and the staff seemed friendly, but my experience was terrible. Routine neglect - missed meds and IVs, delayed orders, wounds not changed, infrequent showers, raw food and scarce snacks - led to dehydration, aspiration pneumonia, a UTI and an emergency hospital transfer; weekend staffing and emergency response were poor. The building is small, expensive, outdated, smells at the entrance, has mice in the halls and lax security; roommate hygiene was an ongoing problem. There are activities and some caring staff, but overall the care was unacceptable and I cannot recommend it.
