The unit serves individuals who require additional skilled care before returning to their home environment or transferring to an alternative setting, such as long-term care, assisted living, or a rehabilitation center. The typical length of stay at the TCU is less than 20 days, with most patients receiving care for one to two weeks. This short-term focus allows the team to support a wide range of recovery needs without lengthy hospitalization.
Patient admissions to the Transitional Care Unit are coordinated by the attending physician. For patients whose doctors are not on the medical staff at Sharon Regional Medical Center, the Unit's Medical Director or another member of the medical staff can assist with admission. The facility is dedicated to a comprehensive, multi-disciplinary approach to patient care, offering a full suite of rehabilitative and clinical services tailored to individual patient needs.
