    Dirty understaffed facility poor care

    I placed my mother here after a stroke; she died two months later and I'm devastated and very concerned. The nurses were often compassionate and some staff knew residents by name, therapy and activities (Elvis acts, church, nails) helped at times, but CNAs were clearly short-staffed, overworked, and sometimes rude. The facility is old and rundown - foul urine odor, mold, peeling paint, rust, cobwebs, mouse traps and stray cats outside - rooms are shared with many beds and very little privacy. Food and meal service were poor and inconsistent, wound care and communication were unreliable (delayed care, abrupt discharges, missing belongings). Management was unresponsive; overall I would not recommend this place without major improvements to staffing, cleanliness, and leadership.
