Fort Sanders Transitional Care Unit (TCU) is a specialized short-term facility designed to support individuals who are transitioning from hospital care to independent living at home. Located on the 9th floor of Fort Sanders Regional Medical Center, the TCU offers an environment that bridges the gap between acute hospital treatment and a return to daily life, providing an essential step in many patients’ recovery journeys. The purpose of the unit is to assist those who no longer require intensive hospital care but are not yet ready to be fully independent, particularly after procedures that require ongoing rehabilitation.
The TCU accepts clients who have already spent a minimum of three days in the hospital and have a specific diagnosis that warrants continued physical or occupational therapy or skilled nursing care. Clients typically come to this unit following orthopedic procedures, such as hip or knee replacements, where comprehensive therapy and personalized attention can greatly impact long-term recovery outcomes. The majority of patients remain in the TCU for approximately 14 to 20 days, allowing them to receive focused rehabilitation and care while preparing for life back in their own homes.
