    Neglectful care and unclean facility

I appreciated many staff being friendly and helpful and some rooms and common areas are large and pleasant, but my overall experience was negative. Care was inconsistent and at times neglectful - residents left in chairs or beds all day, delayed meds, poor transfers and skin breakdown occurred. The facility felt outdated and unclean in places (strong odors, sticky/filthy floors, hygiene issues), menus and cooking were poor, and rehab staffing/quality was uneven. Because of these serious care and cleanliness concerns, I cannot recommend sending a loved one here.

About South Ogden Post - Acute

South Ogden Post - Acute stands out mainly for its focus on medical care and support after hospital stays, providing skilled nursing for folks who need a watchful eye and regular help, and you'll notice right away that care and safety come first, with nurses on the job for 12 to 16 hours each day and a 24-hour call system for peace of mind, whether someone needs help with bathing, dressing, medication, or things like wound care, IV therapy, and diabetes care. The place covers both short-term rehab when people need to get back on their feet after surgery or illness, and long-term care for those with more serious or ongoing needs, and they do offer specialized help for tracheostomy care, cardiac care including CHF and COPD, and orthopedic recovery after things like hip or knee replacement, plus full physical, occupational, and speech therapy programs in a modern 1,600 square foot therapy gym that's set up for busy days.

South Ogden Post - Acute has a large community with both private and semi-private rooms, broad hallways, and private therapy recovery suites, and you'll find plenty of spaces to gather or take it easy, including arts and crafts rooms, a library, fitness areas, movie theater, activity room, and lounges that are all furnished for comfort, along with outdoor walking paths and spots to enjoy the fresh air. Residents don't have to fuss over chores since housekeeping, laundry, and even move-in help are available, and they can rely on transportation for appointments and daily needs.

Meals are served restaurant-style with choices for special diets like diabetes or allergies, and you don't have to eat at a set time thanks to all-day dining options, a nice touch for folks who like to eat on their own schedule. There's a steady rotation of activities, both by the staff and organized by residents, whether it's something creative, social, or wellness-minded, with a spa, sauna, and wellness room included.

For folks paying with Medicare or Medicaid, South Ogden Post - Acute does accept those, and they also help families working on Medicaid-pending status. The facility also uses Nursa, a staffing tool that helps fill open shifts more quickly, which lets nurses, CNAs, and LPNs pick up work as needed so the residents always have someone to help. Overall, South Ogden Post - Acute is built for people who need medical attention along with daily support, and they aim to help each person return home if possible or stay comfortably if more care is needed.
