    Kind staff, serious safety issues

I appreciated the kindness of direct-care staff-Nurse Anne and CNA Gloria were a saving grace-and therapists/aides were attentive; the facility is generally clean, therapy is good, and activities (day spa, movie nights, bingo, church) are nice. Food is often good but inconsistent. Major problems: chronic understaffing, ignored patients, forgotten water and bath assistance, occasional strong urine odors, dementia-care and safety issues (frequent room moves, two-per rooms, stolen clothes, no locks/alarms). Leadership and communication are poor-director Monique was unempathetic and blocked calls, emails get redirected, and administration feels money-focused; until those issues are fixed I can't fully recommend it.

About Louisa Health & Rehabilitation Center

Louisa Health & Rehabilitation Center sits over at 210 Elm Street in Louisa, Virginia, right across from Tractor Supply on Courthouse Road, and offers both short-term rehabilitation and long-term skilled nursing care in a 90-bed facility that's been a part of the community for years now, and the building fills most days with about 85 residents. This center focuses on helping people recover from surgeries, accidents, or illness and provides regular long-term care for folks who need it. The facility features semi-private rooms at $262 per day and private rooms at $286 per day, with services running all day and night under the direction of the administrator, Christina Wiley, and support from Medical Facilities of America.

Nursing staff includes RNs, LPNs, and CNAs, and they provide basic care like bathing, dressing, bringing and collecting food trays, feeding, and checking vital signs. The shift system uses twelve-hour blocks overnight and offers extra pay on weekends for registered nurses, with specialized positions like RN/LPN House Supervisor and activities led by Rochelle Myers. The nurse turnover rate is 50.0%, a bit higher than the state average, and staffing rates at 2.98 hours per resident per day, lower than the Virginia average of 3.8, which lines up with cited deficiencies about not always having enough licensed nurses and staff on shift, though recent reports said there was the potential for harm, not actual harm. Other compliance notes include being flagged for pharmacy service issues, like not always trying non-drug interventions or cutting back doses before keeping people on certain medicines, again with the risk rated as more than minimal but without actual reported harm.

The most recent inspection happened back in January 2023, which found a total of ten deficiencies, some about infection control too, which means some things weren't fully up to regulations, but no harm actually happened. There's also been a delay since then for another official check, now going past the recommended 9-15 month window between inspections. The ownership is with Louisa Holdings LLC, managed by Rybst Central Manager LLC since May of 2021, and includes other entities like AC 2003 Family Trust and Central Bay LLC involved indirectly, so it's a for-profit business with a classic membership structure. Key staff include Monique Bruce, Bradley Gurski, and Shelby Stratton covering primary, contact, and HR roles.

This center's philosophy, "Commitment to Care, Passion for Caring," says a lot about their approach, trying to keep relationships genuine and experiences rewarding for residents, and while they talk about caring and warm-hearted staff, families might notice that services stay focused on meeting daily needs and keeping care consistent, even with those staffing and compliance challenges.

Recovery Map™ and Personal Report Card™ tools allow patients to track progress during their stay, making the process a bit clearer, and the LifeWorks Rehab® Recovery Program, with therapy available seven days a week for two to three hours a day, gives residents access to regular physical and occupational therapy in a modern gym using up-to-date equipment. Staff members are direct employees, not contractors, and coordinate admissions, discharges, and resident supervision. There's health, dental, vision, and life insurance along with paid time off and a 401K for employees. The main purpose here remains nursing, rehab, and long-term care, in a building that's worked to stay within state and federal guidelines, and keeps its doors open to anyone needing skilled nursing and physical therapy in this part of Virginia.

Louisa Health & Rehabilitation Center keeps its information updated and publishes rates and program outlines on its website, serving as part of the larger network of health and long-term care options in Virginia, and while not perfect, they work with what they've got under the circumstances and the rules set by the state.
