    Beautiful facility but inconsistent care

I had a mixed experience. The facility is beautiful, very clean, with great food (fresh fruit, varied menus), lots of activities, a strong memory-care program, and many caring staff who know residents by name and keep families updated. My respite stay exposed problems: a hot, poorly equipped/dirty room, missing clothes and supplies, odors, inconsistent medical coverage, understaffing/turnover, defensive management and poor follow-up - and it felt expensive for the quality. When staffed well the caregivers were exceptional and residents seemed happy, but the uneven care and lack of reliable healthcare made me hesitant to recommend full 24-hour placement.

About Mosaic Gardens Memory Care at Surprise

Mosaic Gardens Memory Care at Surprise sits in Surprise, Arizona, right next to the Surprise Recreation Campus, and serves seniors with memory care needs, especially those living with Alzheimer's or other forms of dementia, and people often remark on the secure, friendly environment they've created over the years, with activities and staff that help residents hold on to abilities and small victories each day, which can mean a lot. The place is managed by Avenir Senior Housing LLLP and holds license AL12542, offering both long-term residence and short-term respite care, and you'll find a focus on safety and comfort here, with a single-story building for easy walking, secured outdoor areas, a lovely patio with green plants, and bright, naturally lit rooms-many folks enjoy personalizing their own space, making it feel more like home.

There are studio and one-bedroom apartments, each with private bathrooms and climate control, which works well whether someone wants more privacy or prefers things simple and cozy, and you'll find the community is pet-friendly if a resident wants to keep a beloved companion close. Mosaic Gardens uses a unique approach where residents live in small groups with similar cognitive abilities, helping reduce confusion and providing support; staff encourages connection, activity, and routine, often arranging life-engagement programs and music therapy based on more than a decade of research, and they've designed special activities that support memory, sensory, social, and motor skills so people stay engaged in ways that fit them best.

The facility employs a nurse on-site 24 hours a day, so round-the-clock care and medication management are handled, and there are also mobile lab services and therapy when prescribed. Staff help with daily activities-like grooming, getting dressed, reminders for meals, and laundry-so residents don't have to worry, plus meals are served in a shared, attractive dining room, with attention to nutrition and mealtime support, and snacks are always available if needed. Community spaces include a game room, chapel for spiritual needs, a beauty salon, sensory gardens to enjoy the outdoors, and well-lit social areas for gathering, with a packed social calendar full of options like chess, bingo, exercise, and creative projects designed to reduce stress and foster enjoyment.

Care plans are tailored to each person with services that match what's needed for dementia or Alzheimer's, focusing on resident strengths and daily routines that encourage independence where possible. Family feedback and resident reviews have given Mosaic Gardens Memory Care a 3.5-star very good rating, and staff make a point to guide their work with F.A.I.T.H.E. values-Fun, Attitude, Integrity, Teamwork, Honesty, and Effort-while the building's design, the secure entry and exits, and the neighborhood-like layout offer both safety and the chance for people to get to know one another. Weekly housekeeping, linens, local transportation, Wi-Fi, and all basic utilities are included, and programs are built not only around support but also fulfillment, trying to help every resident have a good day, feel valued, and stay connected-even as memory changes. The office opens daily, and people often appreciate having choices in floorplans as well as the easy atmosphere and staff who genuinely know how to help folks stay active and comfortable.
