Garden View Post-Acute Rehab sits on Garden View Lane in Baldwin Park, CA, and the first thing people tend to notice is the genuinely friendly and helpful staff, who go out of their way to make everyone comfortable, and while the building might seem a little dated, it stays super clean and feels bright when you walk through. The facility serves seniors needing skilled nursing or rehabilitation after surgery or hospital stays, with both short-term recovery and long-term care options, and has a dedicated group of nurses, therapists, and medical staff available 24 hours to provide assistance with daily life activities like bathing, dressing, and managing medication, as well as things like wound care, IV therapy, stroke rehab, physical therapy, occupational therapy, and specialized care after hip replacements or tracheostomies. The team works closely with residents and their families to make sure each care plan fits the person's needs, and there are activities every day-art programs, games, movies, social events, and outings-that keep people engaged and feeling part of a community, including worship services and a variety of meals, which are said to be quite good, always made from quality ingredients. Apartment choices include studios and companion rooms, and the facility hosts 86 residents on average, out of a licensed 97 beds, and offers special programs like Respite Care for temporary stays. Garden View has a strong focus on therapy and recovery, using both hands-on and advanced rehab methods, and aims to help residents regain as much independence as possible while making their time there as comfortable and safe as they can. Don't expect perfection: the place has had some problems including 38 inspection deficiencies-four related to infection control-and has been cited for not always meeting resident rights or preferences, but most people describe the atmosphere as positive and staff as attentive. With a nurse turnover rate just under the state average and fewer nurse hours per resident each day than the state average, care is still considered good, and the latest Medicare.gov rating is four stars overall. Management comes through Ryan Leet, since February 2023, under Flagstone Healthcare South LLC, and the facility is part of The Ensign Group Inc. There's a focus on privacy, dignity, and service quality, and residents often say staff provide compassionate support, especially during post-surgical or illness recovery. Garden View Post-Acute Rehab is well suited for seniors needing skilled nursing or rehabilitation in a friendly and clean setting, with an emphasis on making people feel cared for and connected, even if some parts aren't the newest or fanciest around.

Good rehab, serious neglect concerns
I had a mixed experience. The facility is clean and attractive, with strong rehab (Manny, Hans), many caring nurses, good activities and supportive therapy - but I also saw dangerous neglect: long call-light delays, ignored bathroom assistance, meals left at bedside, falls with injuries, unresponsive administration/phone and poor transparency. Unless you can constantly advocate or need intensive rehab, I cannot recommend it.
