    Poor staffing, cleanliness, and communication

I had a mixed but mostly negative experience. Direct caregivers (Jelani, Aracely and several CNAs) were kind, professional and attentive, and some rehab/wound care was excellent - but administration, nursing and business office responsiveness was terrible. I repeatedly encountered unanswered calls, rude front-desk staff, long holds, medication errors, missed vitals/toileting, filthy rooms (urine/diapers, roaches, bad odors), understaffing and safety issues that led to ER/ICU visits. Until management fixes staffing, cleanliness, communication and oversight, I cannot recommend this place.

    About Plaza Healthcare Center

Plaza Healthcare Center in Santa Ana, CA, takes care of seniors with all sorts of needs, whether someone needs help with daily activities like bathing, dressing, and taking medicine, or if someone needs skilled nursing, memory care, or even rehabilitation after a hospital stay, and the staff there helps with personal care, medication management, and has nurses around for 12 to 16 hours each day, while supervision is available 24 hours, so someone's always keeping an eye out. Folks get meals in a dining room set up like a restaurant, and the kitchen staff makes sure to offer meals for people with diabetes or other dietary needs, so no one's left out, and for those who want to stay active, there are fitness rooms, a game room, computer center, planned day trips, and even a beauty salon for styling and grooming, plus they keep a small library and outdoor spaces where people can gather or just sit outside. Each room comes furnished, and there are kitchenettes in case someone wants a snack, and laundry, housekeeping, and linen services are all handled, so residents don't have to worry about chores; if someone needs help moving around, there's walking and wheelchair assistance, and personal care assistants can help with bathing and toileting too. There's a mental wellness program, resident-run activities, arts and crafts, education programs, a wellness center, and lots of daily activities to keep the mind and body engaged, plus the staff will arrange for rides to doctor appointments or outings, with both community-run transportation and help setting up rides for non-medical trips. Plaza Healthcare Center also offers specialized memory care for people with dementia or Alzheimer's, providing a safe space, round-the-clock support, and memory activities, and for those recovering from illness or surgery, there are on-site rehabilitation services, like physical, occupational, and speech therapy, with treatment plans made by therapists focused on getting folks back to moving, feeling stronger, and being more independent. The nursing team can handle things like wound care, diabetic care, orthopedic care, respiratory therapy, and subacute care, and there's a podiatrist and occupational therapists who visit, plus Plaza is a Medicare-certified facility, which means they follow certain standards, and they accept long term care insurance too. The center runs every day between 7 a.m. and 8 p.m., and the place has other practical things like Wifi, washers and dryers, safety features, guest parking, and a sprinkler system. Folks can get personal support at all levels, from independent seniors to those who need a lot of help, and Plaza also has respite care so caregivers can take a short break when needed. The team at Plaza Healthcare Center believes in delivering care that helps restore health, keeps people engaged, and supports each resident with respect, all through a team-based approach, and with their connection to Country Villa Health Services, which runs many facilities across California, residents can feel like they're in good hands for the next step in their care.
