    Events are great, care lacking

I enjoy the dinners and frequent events — performers and activities keep things lively and the place is generally clean — but I'm concerned: staff are often slow and inattentive, there aren't enough nurses, care can be poor, there's a lingering odor, I suspect minor theft, and some residents seem neglected in this older facility.

    About Spring Valley Post Acute

Spring Valley Post Acute is a skilled nursing facility that operates as a Continuing Care Retirement Community and has about 106 residents each day, so there's always something going on and enough people to feel a sense of community but not too crowded, and they've got 126 certified beds so there's usually room for new patients, and since they offer post-acute care services, families looking for help after a hospital stay can find options here, and they've also got rehabilitation services, dementia care, pain management, wound and ostomy care, nutrition support, plus palliative, respite, and hospice care for those who need more specialized help. This place is owned by Spring Valley Post Acute LLC, with Thomas Chambers and David Johnson as direct owners, and Meridian Management Services LLC has managed it since December 2013, which means they've been around with the same team for a while now. Spring Valley Post Acute runs as a for-profit limited liability company, so they keep an eye on their business matters, but they also talk a lot about caring for residents like family, building good rapport, and focusing on customized, individual attention for each person-there's an effort here to treat people like whole individuals, even making sure to support mental and emotional wellbeing, not just medical needs. The facility offers services related to hospitalization, nursing home care, home health, in-home care, and they've got something called Convenient Care coming soon, so they've tried to cover different stages and needs that older adults might have. There's an emphasis on age-friendly care and doing their best to provide what older adults need and want, and with an average nurse staffing level of 4.68 nurse hours per resident per day, that's better than the state average, so there's usually someone around to help, but like most places, it's not perfect-Spring Valley Post Acute has 37 deficiencies listed in inspection reports, which include some serious ones, like missing the PASARR screening for mental health or intellectual disabilities, not always providing care in line with orders or residents' wishes, and three infection-related violations, so families and residents should be aware and ask questions about how these issues get fixed. Spring Valley Post Acute does offer a lot of rehabilitation and physical therapy options in their dedicated center and nurses try to get to know each resident to figure out the best care for their particular situation, and while no place is without its problems, the staff here talks about treating residents like family and seems to make an effort to understand what each person needs to feel comfortable, both physically and emotionally.
